The Beaver Dam Unified School District will hold registration for 4 year old Kindergarten on the following dates and times:

- February 24 – 9:00 am to 2:00 pm
BY APPOINTMENT ONLY
Registration for all 4K sites will be held BY APPOINTMENT ONLY at the Educational Service Center (705 McKinley St). The appointment sign up calendar will be available on the BDUSD.org website starting February 4, 2022.
Children 4 years old on or before September 1, 2022 are eligible for 4-year-old kindergarten.
Parents registering a student for 4-year-old kindergarten should bring a certified copy of their child’s birth certificate, their child’s immunization records, $30 materials fee (cash or check only) and proof of residency to registration. Proof of residency requirements have changed. Please see the district website ( www.bdusd.org ) for acceptable proofs of residency.
Masks are required. Only one parent should attend registration. Children should not attend.
Registration forms are available on the Beaver Dam Unified School District’s website (www.bdusd.org) for parents that wish to print them and bring the completed forms to registration.
